Pregnancy Outcome after Laparoscopic Surgery for Tubal Pregnancy

Abstract
The aims of this study are to compare pregnancy rates after laparoscopic surgery for tubal pregnancy in normal and infertile patients, and to determine the factors which affect pregnancy outcome after laparoscopic surgery. Between January. 19-89 and August 1992, 461 patients with tubal pregnancies were treated by laparoscopic surgery. 145 patients desiring subsequent pregnancy were included and followed by phone. Surgical techniques included for analysis are laparoscopic salpingectomy and salpingostomy. In a retrospective analysis, we examined crude pregnancy rates and life-time table analysis of cumulative pregnancy rates.
No difference in pregnancy rates was observed after laparoscopic salpingectomy and salpingostomy, also salpingostomies were almost three times as likely as salpingectomy to become repeat ectopic pregnancy. 7 of 141(5.0%) treated salpingostomy had persistent trophoblastic activity. Normals had a significantly higher pregnancy rate than infertility patients, independent of types of surgery.
In conclusion. our study shows that successful pregnancy after tubal pregnancy is related to previous history of infertility and risk factor rather than types of surgery to treated ectopic events. and that laparoscopic salpingostomy may be no better than laparoscopic salpingectomy.
